Benefit and Description

Blue Cross Blue Shield ND-Comp Choice 250 Health and Dental Insurance
Employees must fill out the Group Policy Application and return it to Human Resources within 30 days of their hire date. Insurance is effective the first or the 16th of the month following 30 days of eligible employment. If an employee does not enroll for coverage he/she is not eligible to enroll again until December, with a coverage effective date of January 1.

Standard Short Term Disability Insurance
Fort Dearborn Life short-term disability (STD) insurance is available as a voluntary benefit through payroll deduction. If disabled, STD payments commence on the eighth day for physical disease, injury, pregnancy, or mental disorder. STD payments begin immediately for accidental injury. Pays 60% of base salary up to $500/week.
Standard Long-Term Disability Insurance
Fort Dearborn Life long-term disability (LTD) insurance is effective the first or the 16th of the month following 30 days of eligible employment. If disabled, LTD payments commence after 90 days of ongoing physical disease, injury, pregnancy complications, or mental disorder. Pays 60% of base salary up to $5000/month.
Life Insurance
Fort Dearborn Life
life insurance is effective the first or the 16th of the month following 30 days of eligible employment. Employees receive life insurance coverage the equivalent of 1X annual salary, up to $50,000.
Vision Insurance
Avesis vision insurance is available as a voluntary benefit through payroll deduction. Vision insurance is effective the first or the 16th of the month following 30 days of eligible employment.
American Funds 401(k) Plan
Employees are eligible after working 1000 hours. Enrollment is quarterly. Automatic enrollment is also available at a default contribution of 2%. Employer match is discretionary.

Discovery Benefits Flexible Spending Accounts
Employees must fill out the enrollment application and return it to Human Resources within 30 days of their hire date. Flex spending account is effective the first pay period following 30 days of eligible employment. Employees can pay their out-of-pocket medical and/or dependent care expenses through flex spending account. Maximum contribution amount for medical is $3000 and dependent care is $5000.
